Morning After Pill

Levonelle one step is commonly known as 'the morning after pill'. It is used to reduce the chances of becoming pregnant after unprotected sexual intercourse. Levonelle one step tablets contain the active ingredient levonorgestrel, which is a synthetic derivative of the naturally occurring female sex hormone, progesterone.

EllaOne is a revolutionary new emergency contraceptive pill which can prevent unwanted pregnancy up to 5 days after unprotected sex the failure of other contraceptives – two days more than the standard morning-after pill.   About the condition


Levonelle


Levonelle 1500 is an emergency contraceptive that can be used within 72 hours (3 days) of unprotected sex or if your usual contraceptive method has failed.

Not to be used in

           * Pregnancy
           * Women who had unprotected sex more than 72 hours previously, or whose menstrual period is late.
           * This medicine is not recommended for women with severely decreased liver function.

This medicine should not be used if you are allergic to one or any of its ingredients. Please inform your doctor or pharmacist if you have previously experienced such an allergy. If you feel you have experienced an allergic reaction, stop using this medicine and inform your doctor or pharmacist immediately.

Cautions

           * Crohn's disease
           * Rare hereditary problems of galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption.
           * Regular contraceptive pills should not be stopped after the mornig after pill has be taken
           * The morning after pill should not be used more than once in a menstrual cycle.
           * The morning after pill is for use after intercourse, not before.
           * The morning after pill is for occasional emergency use. There are better methods of contraception for routine regular use.
           * There is no increased risk of ectopic pregnancy after taking the morning after pill. However ectopic pregnancy can still occur. Seek medical advice if there is abdominal pain and vaginal bleeding 4-5 weeks after taking the morning after pill as there is a chance these are the symptoms of an ectopic pregnancy.
           * If a woman becomes pregnant after taking the morning after pill, or was already pregnant when it was taken, the baby will not be effected.

Warnings


           * Levonelle one step cannot be sold from pharmacies to girls under 16 years of age except in exceptional circumstances. Girls under 16 who need the morning after pill are advised to consult a doctor, family planning clinic or casualty department at a hospital. Some minor injuries departments at hospitals also provide the morning after pill.
           * Your pharmacist will ask about your last period and when you last had sex in order to make sure this medicine is suitable for you. This medicine can be used at any point during your menstrual cycle, apart from if your period is late. If your period is late, or your last period was more than five days late or unusually light or heavy, you should tell your pharmacist, as you may already be pregnant and this medicine will not work.
           * This tablet should be taken as soon as possible, preferably within 12 hours and no later than 72 hours after unprotected sex (see above). If you are sick within three hours of taking the tablet, another one should be taken immediately. You should consult your doctor, pharmacist, family planning clinic or casualty department for advice and to obtain another tablet.
           * This medicine might make your next period earlier or later than expected by a few days. If your next period after taking this medicine is more than seven days late, or is abnormal in any way, you should consult your doctor for a pregnancy test.
           * The morning after pill does not provide continued contraception to prevent pregnancy after you have taken it. It is recommended that after taking it, you use a barrier method of contraception such as a condom or cap, until your next period comes. This is also recommended even if you are also taking the pill. You should talk ask your doctor, pharmacist or family planning clinic for advice about regular methods of contraception.
           * This medicine should only be used as an emergency measure. It is not recommended for repeated use, as it is possible that it can disrupt your menstrual cycle.
           * This medicine will not always prevent a pregnancy. If you do become pregnant after taking this medicine there is a possibility that the pregnancy will be ectopic (ie occuring in the fallopian tubes rather than the womb). For this reason, you should consult your doctor if you experience any sudden or abnormal abdominal pain after taking this medicine, and if your next period is shorter or lighter than your normal period.
           * Emergency contraception does not protect you from sexually transmitted infections.

About one in a hundred women vomit after taking the morning after pill. If vomiting occurs within three hours of taking the tablet the dose needs to be repeated.


Ellaone


Ellaone tablets contain the active ingredient ulipristal acetate, which is a synthetic derivative of the naturally occurring female sex hormone, progesterone. It is used to reduce the chances of becoming pregnant after unprotected sexual intercourse.

It is not fully understood how this medicine prevents pregnancy. It is thought to work by preventing ovulation and fertilisation and also by altering the lining of the womb, depending on which stage of the menstrual cycle the woman is at.

In a woman's normal menstrual cycle, an egg matures and is released from the ovaries (ovulation). The ovary then produces progesterone, which prevents the release of further eggs. Ulipristal, tricks the body processes into thinking that ovulation has already occurred, as it provides high levels of synthetic progesterone. This prevents the release of eggs from the ovaries.

Ulipristal also increases the thickness of the natural mucus at the neck of the womb (cervix), making it more difficult for sperm to cross from the vagina into the womb. By preventing sperm entering the womb, successful fertilisation of any egg that is there is less likely.

Not to be used in

           * Pregnancy.
           * Women who had unprotected sex more than 120 hours previously, or whose menstrual period is late.
           * This medicine is not recommended for women with severely decreased liver function.
           * The safety and efficacy of this medicine in girls under 18 years of age has not been studied by the manufacturer.

About Levonelle

Levonelle 1500 is an emergency contraceptive that can be used within 72 hours (3 days) of unprotected sex or if your usual contraceptive method has failed.

Levonelle 1500 contains a synthetic hormone like substance called levonorgestrel. It prevents about 84% of expected pregnancies when you take it within 72hrs of having unprotected sex. It will not prevent a pregnancy every time and is more effective if you take it as soon as possible after unprotected sex. It is better to take it within 12hrs rather than delay until the third day.

Levonelle 1500 is thought to work by:

           * stopping your ovaries from releasing an egg;
           * preventing sperm from fertilising any egg you may have already released; or
           * stopping a fertilised egg from attaching itself to your womb lining.

Levonelle 1500 can only prevent you becoming pregnant if you take it within 72hrs of unprotected sex. It does not work if you are already pregnant. If you have unprotected sex after taking Levonelle 1500, it will not stop you from becoming pregnant.

Side Effects

Medicines and their possible side effects can affect individual people in different ways. The following are some of the side effects that are known to be associated with this medicine. Because a side effect is stated here, it does not mean that all people using this medicine will experience that or any side effect.

           * Nausea
           * Vaginal bleeding after taking the tablet (not related to your normal period)
           * Abdominal pain
           * Fatigue
           * Early or delayed start of next menstrual period
           * Menstrual spotting or irregular bleeding
           * Headache
           * Diarrhoea
           * Dizziness
           * Breast tenderness

Uses of Levonelle

Levonelle one step tablets contain the active ingredient levonorgestrel, which is a synthetic derivative of the naturally occurring female sex hormone, progesterone.

Dosage

Always use Levonelle 1500 exactly as your doctor has told you. You should check with your doctor or pharmacist if you are not sure.


           * Take the tablet as soon as possible, preferably within 12 hours, and no later than 72 hours (3 days) after you have had unprotected sex. Levonelle 1500 can be taken at any time in your menstrual cycle assuming you are not already pregnant or think you may be pregnant. Do not chew the tablet, swallow it whole with water. Do not delay taking the tablet. The tablet works better the sooner you take it after having unprotected sex.
           * If you are already using a regular method of contraception such as the contraceptive pill, you can continue to take this at your regular times.

If another unprotected intercourse takes place after the use of Levonelle 1500 (also if this is during the same menstrual cycle), the tablet will not exert its contraceptive effect and there is again the risk of pregnancy.

What to do if you are sick (vomit)
If you are sick (vomit) within three hours of taking the tablet, you should take another tablet. You will need to contact your pharmacist, doctor, practice nurse or family planning clinic immediately for one more tablet.

After you have taken Levonelle 1500
After you have taken Levonelle 1500, if you want to have sex, and are not using the contraceptive pill, you should use condoms or a cap plus spermicide until your next menstrual period. This is because Levonelle 1500 won’t work if you have unprotected sex again, before your next period is due.

After you have taken Levonelle 1500, you are advised to make an appointment to see your doctor about three weeks later, to make sure that Levonelle 1500 has worked. If your period is more than 5 days late or is unusually light or unusually heavy, you should contact your doctor as soon as possible. If you do become pregnant even after taking this medicine, it is important that you see your doctor.

Your doctor can also tell you about longer-term methods of contraception which are more effective in preventing you from getting pregnant.

If you continue to use regular hormonal contraception such as the contraceptive pill and you do not have a bleed in your pill-free period, see your doctor to make sure you are not pregnant.

Your next period after you took Levonelle 1500
After the use of Levonelle 1500, your period is usually normal and will start at the usual day; however sometimes, this will be a few days later or earlier. If your period starts more than 5 days later than expected, an ‘abnormal’ bleeding occurs at that time or if you think that you might be pregnant, you should check whether you are pregnant by a pregnancy test.

If you take more Levonelle 1500 than you should
Although there have been no reports of serious harmful effects from taking too many tablets at once, you may feel sick, actually be sick (vomit), or have vaginal bleeding. You should ask your pharmacist, doctor, practice nurse or family planning clinic for advice, especially if you have been sick, as the tablet may not have worked properly.

About Ellaone

ellaOne is basically a synthetic version of the female hormone progesterone. When a woman ovulates, the body releases progesterone which tells the ovaries to stop releasing more eggs. Taking the ellaOne contraceptive pill tricks the body into thinking that ovulation has already occurred – this basically delays the release of the egg, preventing pregnancy. In the event that an egg has already been released, ellaOne works by increasing thickness of the mucus at the cervix. This additional effect reduces the chances of sperm entering the womb and also reduces the chances of pregnancy.

Side Effects

Medicines and their possible side effects can affect individual people in different ways. The following are some of the side effects that are known to be associated with this medicine. Because a side effect is stated here, it does not mean that all people using this medicine will experience that or any side effect.

           * Abdominal pain.
           * Various types of infections, such as nose or throat, urinary tract infection.
           * Changes in mood.

Dosage

ellaOne should be taken as soon as possible, and no longer than 5 days after unprotected sex or after other methods of contraception have failed. It can be taken before, during or after a meal. Should you vomit within 3 hours taking ellaOne, you should take another dose.

Important Notes

It is important to tell your doctor or pharmacist what medicines you are already taking, including those bought without a prescription and herbal medicines, before you start treatment with this medicine. Similarly, check with your doctor or pharmacist before taking any new medicines while taking this one, to ensure that the combination is safe.

The morning after pill may be less effective if you are taking any of the following medicines, because these medicines speed up the breakdown of levonorgestrel by the liver:


           * antiepileptic medicines such as carbamazepine, oxcarbazepine, phenytoin, phenobarbital, topiramate or primidone
           * barbiturates
           * griseofulvin
           * nevirapine
           * protease inhibitors for HIV infection such as ritonavir
           * rifamycin antibiotics such as rifabutin or rifampicin
           * the herbal remedy St John's wort (Hypericum perforatum).

If you are taking any of these medicines regularly, you may be advised to take two doses of this medicine, one tablet immediately and another twelve hours later (ie a total dose of 3mg). This is an unlicensed use of the medicine. An alternative would be to have a coil fitted as emergency contraception, as this is not affected by the medicines you are taking. You should discuss these options with your doctor, pharmacist or family planning clinic.

This medicine may increase the blood level of ciclosporin (an immunosuppressant).

This medicine may decrease the blood level of the antiepileptic medicine lamotrigine.
